Before a cadet can attend an activity they must be an official Civil Air Patrol Member, which means that they have an identification number and are recognized by CAP National Headquarters. In addition they must complete both a Form 31 and a Form 60 and have it approved by the designated deadline date. Download and type your information in the given fields, print the forms and bring them to the meeting. Your forms must first be approved by the Administration & Personnel Officer, 1st Lt Twila Hoffman, and then signed by the Squadron Commander or Deputy Commander for Cadets. Form 31
